Terms of Service

These Terms of Service ("Terms") govern your use of the Evoq Health platform operated by NUMO at evoq.health. By using Evoq, creating an account, or enrolling as a patient, you agree to these Terms.
1. Eligibility
You must be at least 18 years old to use Evoq Health. The platform is intended for enrolled patients participating in a clinician-supervised care program.
2. Service description
Evoq Health provides clinician-guided health optimization tools including protocol planning, AI support, wearable and lab integrations, and care-related messaging.
Evoq is not emergency medical care. If you think you are experiencing a medical emergency, call 911 or local emergency services immediately.
3. Accounts and user responsibilities
You are responsible for safeguarding your account credentials and for activity performed through your account. You agree to provide accurate profile and health information.
4. Healthcare-related disclaimer
Evoq supports, but does not replace, direct professional medical judgment. Recommendations should be interpreted in the context of your licensed clinician's guidance.
5. SMS messaging terms
By providing your number and completing onboarding, you consent to receive care-related SMS messages from Evoq, including Eliza (Dr. ElizaMD) coaching messages and automated care notifications.
Current code paths include onboarding welcome SMS, Oura workout prompts, morning sleep summaries, HRV alerts, Fullscript lab and supplement notifications, and appointment scheduling or status updates. Message and data rates may apply.
6. Acceptable use
You agree not to misuse the platform, access systems without authorization, transmit harmful or fraudulent content, or interfere with service security and operations.
7. Intellectual property
Evoq platform content, software, branding, and materials are owned by NUMO or its licensors and protected by applicable intellectual property laws.
8. Third-party services
Evoq may integrate with services such as Oura, Calendly, Fullscript, Twilio, and OpenAI-enabled infrastructure. Use of those services may be subject to their own terms and policies.
9. Limitation of liability
To the extent permitted by law, NUMO is not liable for indirect, incidental, special, or consequential damages related to use of the platform.
10. Termination and service changes
We may suspend or terminate access for misuse, policy violations, or legal/compliance reasons. We may also update, modify, or discontinue parts of the service as needed.
11. Changes to these terms
We may revise these Terms periodically. Continued use of Evoq after updates means you accept the revised Terms.
12. Governing law and contact
