Evoq Health ("Evoq," "we," "us," or "our") is a clinician-supervised health optimization platform operated by NUMO. This Privacy Policy explains how we collect, use, share, and protect information from visitors and enrolled patients using evoq.health.
1. Information we collect
We collect account information (name, email, phone, date of birth), care profile inputs, wearable data (including Oura), lab-related data, appointment data, and communication logs needed to deliver care.
We also collect technical usage data such as IP address, browser details, and device identifiers to operate and secure the platform.
2. How we use your information
We use data to provide your care protocol, coordinate care-team communication, deliver health notifications, monitor protocol adherence, and improve platform reliability and safety.
We also process data to meet legal and regulatory requirements.
3. SMS messaging consent and notifications
If you provide a mobile number, Evoq may send care-related SMS messages that support your use of the platform and care workflow.
Based on current system behavior, these messages can include onboarding welcome messages, Oura workout prompts, morning sleep summaries, HRV recovery alerts, Fullscript lab and supplement notifications, and appointment scheduling or status updates.
Message timing and volume depend on your activity, clinician actions, and enabled integrations. Message and data rates may apply.
4. How we share your information
We do not sell personal or health data. We share information only with your authorized care team and approved service providers required to deliver Evoq services.
Service providers may include Twilio (messaging), Calendly (appointments), Fullscript (lab/supplement workflows), Oura (wearable data), and OpenAI-enabled AI infrastructure.
5. Data security and protection
Evoq applies HIPAA-aligned safeguards including access controls and encryption for data in transit and at rest.

6. Data retention
We retain data while your account is active and for legally required periods. You may request deletion where applicable by contacting support.
7. Your privacy rights
Depending on your jurisdiction, you may request access, correction, deletion, or restriction of your personal data by contacting support.
8. Children
Evoq Health is intended for adults 18 years or older. We do not knowingly collect data from children.
9. Changes to this policy
We may update this policy from time to time. Material updates will be communicated within the platform or through direct notifications.
